Insurance Waiver: I hereby release and save harmless, the Sturgeon Lake Sailing Club, its staff, members, and directors from any claim in respect of any incident, accidental or otherwise, to the above participant(s) in the Junior Sailing Program. I acknowledge having read, understood and agree to indemnify the Club in respect of any claim with this waiver release. In case of emergency: I understand that the Head Instructors (or designate) in an emergency will act as my agent to engage medical attention and /or hospitalization if it is deemed necessary. Concussion Protocol: I and my parent/legal guardian confirm that I have reviewed the provided Concussion Resources and I acknowledge and commit to the Ontario Sailing Concussion Code of Conduct. Sailor Expectations: Posted on the program page of the SLSC website is the Sailor Expectations document. I acknowledge that I have read and understood the guidelines, and will review them with the sailor. Damage to SLSC Property: In the event that my child causes readily avoidable or deliberate damage to SLSC property, I acknowledge and agree that I may be responsible for the cost to remedy such damage Consent to Receive "Commercial Electronic Messages": I agree that SLSC may send me email, or similar electronic communications, in connection with the programs and activities of the club. I may opt out of receiving further emails at any time. Photo Release and Indemnity: SLSC, its employees and representatives is authorized to take photographs of my child or me at its programs and events for publicity and promotional purposes only. I, as parent or guardian, hereby consent to the publication and use by SLSC of any such photographs for publicity and promotional purposes on the SLSC web site, social media platforms, or other marketing initiatives. As guardian I accept this waiver.
